I acknowledge and agree to the following terms and conditions before engaging in any activities associated with OurFamilyJourney LLC, doing business as Change the Stigma:

1. Age Requirement: I certify that I am at least 21 years of age.

2. Consultation with Physician: I affirm that I have consulted with a physician regarding the safe and appropriate use of cannabis products.

3. Non-Operational Agreement: I agree not to operate any vehicle or machinery while under the influence of cannabis products.

4. Legal Compliance: I understand and agree to comply with all applicable local, state, and federal laws and regulations regarding the use of cannabis products.

5. Liability Release: I hereby release, waive, and discharge OurFamilyJourney LLC, its agents, employees, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation in activities associated with Change the Stigma, including but not limited to the use of cannabis products.

6. Assumption of Risk: I acknowledge that the use of cannabis products involves inherent risks, including but not limited to health risks and legal risks, and I voluntarily assume all risks associated with such use.

7. Indemnification: I agree to indemnify and hold harmless OurFamilyJourney LLC, its agents, employees, and affiliates from any and all losses, damages, liabilities, costs, or expenses, including reasonable attorney fees, arising out of or related to my use of cannabis products or my breach of any terms of this agreement.

8. Acknowledgment of Understanding: I acknowledge that I have read this disclaimer form in its entirety, that I understand its terms and implications, and that I agree to be bound by its terms.
